Affidavit of Indigency – If you are charged with a criminal matter that could potentially result in incarceration, you may represent yourself, hire an attorney or ask for an attorney to be appointed. This decision is normally made at your first court appearance. However, if you would like to be considered for court appointed counsel prior to your court date or if you are already represented by a court appointed attorney and have been charged with an additional crime, print and fill out this form completely and accurately if you would like for the Public Defender to be assigned to your case. Bring this completed form to the Public Defender’s Office for review. Our office will review the form and determine if you qualify. Please note that if you are denied by our office for court appointed counsel, you may ask a judge to determine your qualification on your court date.
